What is the Fort Smith Choice Plan?
The Fort Smith Choice Plan is an open access point-of-service (POS) plan. Employees have a choice of two exclusive networks within the Fort Smith Choice service area, Health Point Network and Sparks PremierCare. All family members are assigned to the provider network selected by the employee at the time of enrollment. Network changes can be made only during open enrollment for the following contract year. The provider network selected at the time of enrollment is printed on each member’s ID card. In-network services must be provided by a provider within this network. On the Fort Smith Choice Open Access POS Plan, the member is responsible for a deductible, copayment and/or coinsurance for services received. The deductible, copayment and coinsurance amounts for specific services are listed in the Benefit Summary for your plan.
