Program sponsors use Form DS-3037, “Application for Redesignation and/or Amendment” to apply to the Department of State for redesignation of exchange visitor program sponsor status, or apply to the Department of State for an amendment of their current exchange visitor program (for example: to apply for a new exchange visitor category).
