What is the Foreign Service Benefit Plan’s Disease Management Program for members who reside in foreign countries?
The Foreign Service Benefit Plan and Coventry Health Care, the Plan’s underwriter, offer disease management services to our members who reside in foreign countries. This is an exclusive arrangement that the Plan has with Coventry and we are very pleased to offer this to our members. The program is tailored specifically to meet the needs of members with diabetes, coronary artery disease, asthma, chronic obstructive pulmonary disease (COPD) and heart failure. The program provides basic information and support to members via our secure e-mails. We send educational materials and notifications about the member’s condition. Participation is voluntary and confidential. Services include: disease specific educational materials, professional online resources and direct, personalized consultation with case managers. The Plan contacts members who have the diseases referenced above individually on how to sign up for the program. Members must contact the Plan at FSBPhealth@cvty.com, a secure e-mail
