This program is for individuals and families who are eligible to enroll in or are currently enrolled in their employer sponsored health insurance. In order to be eligible for this program, you must meet income/resource eligibility requirements for the Family Health Plus Program. The following criteria also must be met: • must be a resident of New York State and between 19 and 64 years of age; • must be a citizen of the United States or a Medicaid eligible qualified alien; • must not be eligible for Medicaid based on income and/or resources; • and eligible for, or enrolled in, employer based health insurance. Employer-based insurance must have a standard scope of services and also must be determined qualified and cost effective by the Department of Social Services. The individual or family will receive comprehensive health care coverage through the health insurance company their employer participates with.
