Whole wheat is only talking about wheat and whole grain is talking about any type of grain, like rye, etc. or a combination of grains.

whole wheat is just that, the whole grain of wheat is ground up to make whole wheat flour. whole grain sort of means the same thing except it can include other grains, like barley, oats, flax, etc.
