• The Stipulated Plan (VR1) is a stipulated agreement plan that is formulated by the injured worker and vocational rehabilitation counselor and agreed upon by all parties. If all parties sign the plan, a Commission order will be issued. If any disagree, the plan will be filed for record. • The Service Plan (VR10) is formulated solely by the injured worker and vocational counselor without the consent or agreement of any other party. The Service Plan must be submitted to the Commission; the Commission will send the plan to the employer/insurer with a 15-day letter and controversion (disapproval) form requiring an approval or disapproval with 15 days. If parties agree, a Commission order will be issued. Any disagreement to the plan with result with the case being set for a hearing.
