What is the difference between the Security Agreement and the Security Affidavit?
The Security Agreement form, which coordinators sign in agreement to keep test materials secure, is to be filled out by the CDC and kept on file in the district office. The Security Affidavit form is a separate statement of security regarding the content of the test, and is required to be filled out and signed by each district staff member who works with the CELDT.
