The Iraq supplemental is required for all visa applicants in Iraq. The DS-157, DS-158 and Iraq supplemental forms can be found on our website at http://iraq.usembassy.gov/iraq/visa_forms.html. They do not exist electronically, meaning that applicants need to print them out and then fill them out by hand.
