What is the difference between an MD/PhD program, a combined degree program, and a Medical Scientist Training Program (MSTP)?

None. Programs designed to train physician-investigators go by all of these names. For the most part, the terms are interchangeable, although at some schools “combined degree” programs can include MD/JD and MD/master’s degree programs as well. NIH uses the term “MSTP” to refer to schools that have been competitively awarded special training funds to help support MD/PhD candidates. There are currently 39 NIH-funded MD/PhD programs.
