A NYS DOH form 2832 certificate is obtained through us, and is issued to individuals who successfully complete our Model Accreditation Courses. The 2832 is then sent to Albany with a DOL Hard Card Application. The certified individuals must be employed by a licensed contractor in order to perform work in NYS.
