What is the difference between an affidavit and a questionnaire?
• Affidavit – Form for groups with 6 or more subscribers enrolled on the group health plan. This form must be fully completed, signed and dated by an authorized group executive. **Questions 3 – 6 must equal question 2. • Questionnaire – Form for groups with 5 or less subscribers enrolled on the group health plan. This form must also be fully completed, signed and dated by an authorized group executive. **You must return current tax documentation for each employee enrolled on the group plan and proof of other coverage for each full-time employee that is declining coverage with your group when you receive this questionnaire.
