A revocation is a beneficiary’s choice to no longer receive hospice care. The beneficiary must sign a revocation form indicating the date they wish the revocation to become effective. In both situations, the hospice must indicate that the hospice care is ending by using an occurrence code 42 on their claim with the date of discharge or revocation.
