An HMO plan and ePPO plan require that treatment be rendered by a dentist that is on the provider panel for that specific plan. Services received by a dentist that is not on the provider panel of that plan are not covered. A PPO dental plan allows the benefit option of going to a dental provider that is in network or to a provider that is out of network for that plan. Your out-of-pocket expenses are higher when you use a dental provider that is outside of the network.
