What is the definition of physically present?
Physically present is defined as located in the same room as the patient and/or provides a face-to-face service. The following guidelines are provided to further clarify the teaching physician documentation requirements: 1. Resident note and no attending note- not billable 2. Resident note with attending co-signature or “seen & agree” not billable 3. Resident note with “attending also examined patient” and no attending note – not billable 4. Resident note with “attending also examined patient” and attending co-signature not billable 5. Attending note only-billable 6. Resident note and independent (no linkage) attending note-billable based on attending note only 7. Resident note and documentation of three key elements as performed with or by the attending and attending co-signature with an accompanying attending note-billable using both notes as support for assigned code 8. Resident note and attending note with linkage to three key elements, both notes may be used as support for the ass
