What is the Customer information sheet?
The Customer information sheet is an ITS Site License form that must be completed and sent to ITS along with the requisition. The form is very simple and will help the site license office process your order properly. This form is also important because it allows you to designate someone else to pick-up your software or license. If it is left blank, the site license office will not allow your order to be picked up even if you sent that person. This will protect your investment in the site license software. If no Customer information sheet is attached to the requisition, the processing of your order might be delayed.
