Burial Permits are $11.00 each. If you are changing location of the remains within the first year after death, there is no charge to apply the Affidavit to Amend a Record. If it has been more than one year since the death, the charge by the State of California is $20.00, which includes a copy of the death certificate along with the affidavit showing the change.
