What is the Corporate Compliance Annual Certification (CCAC)?
Form which provides an opportunity to verify that potential compliance-related items have been disclosed to the Hospitals & Clinics Integrity Office. OHSU hospitals and clinics employees in a department manager or director position are required to complete the form as part of the Big Brain CoI module. Please contact the H&C Integrity Office for more information; 503-494-2133. Please note that because of the addition of the CCAC form, 2006 records will show as “incomplete” for the overall CoI module status.
