Some good old fashion elbow grease and getting down on your hands and knees is really the best way to get it really clean. You can use a simple bleach water solution with a hard bristle brush. A product like OxyClean will work great for this too. You can make a paste with it or more of a liquid (to use a squirt bottle). If you want to be a bit more “eco-friendly” make a paste with warm water and baking soda. Use a small grout brush to scrub it a bit, let it sit for a few minutes, scrub some more, then rinse clean. After you get it ncie and clean and rinsed and dried thoroughly, you can apply a grout sealer to protect it. I have to do this to my entire kitchen floor this week. I am not looking forward to it, but I will enjoy the nice look of clean grout! Good Luck!
