What is the authorization form and why do I need to sign it before I can earn points through Caremark?
The authorization form is required by the Health Insurance Portability and Accountability Act (HIPAA) to protect the privacy of you and your family members covered by Caremark prescriptions benefits. These signatures allow Caremark to review your health information in order to assign rewards points based on your eligible transactions and to provide that information to Bank of America. No personal information will be shared with the Bank, only the number points you have earned will be transmitted.
