Designed by a MD, the Apollo PHR is an organized, up to date, centralized, complete set of medical records that you can enter online. This information is available to you and your designated caregivers at any time. Your unique, personalized record consists of information entered online such as, personal identifying information, emergency, insurance, pharmacy contact information, medical and surgical diagnosis, medications and resuscitation status.
