What is the Alcohol/Drug Evaluation Diagnostic Evaluation Referral form (DSHS 9-638)?
The Diagnostic Evaluation Referral form is intended for use by the courts to refer an offender for either assessment or treatment. It is an optional form, available for the courts to use in the event no other referral system is in place. The Diagnostic Evaluation Referral form is the clients ticket to assessment/treatment. It should never originate from an assessment/treatment agency.
