What is the Affidavit of Identiy?
The Affidavit of Identity is simply a form that you should download, print and take to a notary. This form verifies your identity through a third person and is required to receive a certificate in our courses. It can be faxed to our office at 770-792-6775. Once you have logged into your courses, you will be able to see if we have received your Affidavit. It will show in the top right corner of your screen. You will be able to take and complete your courses if we have not received the affidavit, BUT you cannot receive your certificate of completion until we receive it. The sooner you send your affidavit, the faster we process your certification.
