What is the Accommodation Memo?
The Accommodation Memo is a document which lists the accommodations that will enable you to equitably participate in academic life. It is developed with you once your documentation has been determined sufficient for services through our office. It does not contain your diagnosis, but does include contact information for our office, in the event that instructors have questions. • How do I get my Accommodation Memo changed later if it doesn’t meet my needs? The memo is unique to your needs and is a flexible document, allowing for revisionsadditions, deletionsas needed. You will be responsible for scheduling an appointment with a Disability Specialist to discuss changes to your memo. Sometimes, this request will also prompt a need for more documentation. • How often should I come to the PSD office? It is recommended that you pick up Accommodation Memos for each professor/instructor the first week of class. Then make appointments to talk with each professor/instructor during his or her off
