What is ShareWISE?
TCRCs ShareWISE project coordinates communication and information for social networks[1] comprised of clinical professionals, non-clinical volunteers and family caregivers to improve the long term health of underserved citizens. By coordinating care plans that extend beyond organizational boundaries, ShareWISE builds personalized care teams around each individual patient. These private little social networks cooperate to educate patients about access to available services, logistics and management of chronic conditions or medical therapies. Overall, underserved patients receive better care, prevent acute problems that come from poorly managed chronic conditions and become connected participants in their own health, which reduces total costs on the system.
