What is satisfactory evidence that the signer is who s/he says s/he is?
Acceptable identification includes a current identification card or document issued by a federal or state governmental entity containing a photograph and signature of the signer. The notary may also accept a sworn statement of a credible witness who is personally known to the notary and who states that he or she knows the signer.
