Ans: To get a copy of your CDH, phone or e-mail us with the following information: Command: Name/UIC. Period requested. Reason for request. E-Mail/Mailing address. To receive your PDH, mail a letter of request on command letterhead to: Commander Naval Safety Center, Attn: Code 37, 375 “A” Street Norfolk, Virginia 23511-4399. Multiple personnel requests can be made on the same letter. We will need: Diver(s): Name / rate / SSN. Period requested. Reason for request. Mailing address. Retired or discharged personnel requesting a (PDH) may submit requests as a notarized letter, or a letter with the following unsworn statement: “I declare under penalty of perjury, under the law of the United States of America, that the foregoing is true and correct.” The declaration must then be signed and dated.
