A. Athletics is offered to 7th and 8th grade students. Physical clearance forms (3-part form) for Middle School sports can be picked up by a student or adult in the Administration office. Bring the form with you for your physical for them to complete their portion to clear the student for sport activities. The Emergency Release and Code of Conduct form need to be completed by both parent and student. Complete packet needs to be returned to the office prior to the beginning of any team sport activity (NO partial packets will be accepted). We recommend this packet being completed as close to the beginning of the school year as possible, the earlier the better (it is sometimes difficult to get a doctors appointment in time to complete the packet). In addition: Students need to maintain a C or 2.0 grade point average with no F grade on their most recent trimester report card or progress report in order to participate in extra-curricular activities and athletics.
