The effective date and the expiration date of the policy period are required in these fields of the FSBA audit file in a mm/dd/yyyy format. Note that the Data Call instructions, which is available on-line at www.fsba.state.fl.us/fhcf under “Insurer Information”, require the audit file use a two-digit yy. The FSBA prefers the audit file date format for year to include four digits.
