What is required in order for the Office of International Student and Scholar Services (OISSS) to issue a DS-2019 form?
In order to issue the DS-2019 immigration document, OISSS must receive from the potential J-1 Exchange Visitor: 1) *A CV – (can be emailed) 2) *A copy of the passport information page (with name and birth date) 3) A completed Personal Information Form that is sent to the visiting scholar via email from the UNC-CHAPEL HILL department. 4) ORIGINAL financial documents for any funding from an organization besides UNC-CHAPEL HILL or for personal funding. * Emailed scanned copies of those documents marked with a * are acceptable. These documents should be sent to the UNC-CHAPEL HILL sponsoring department so that they can submit them along with other required forms to OISSS in one package. If OISSS has questions regarding your submitted documents, you will be contacted (so please provide an email address on the Personal Information Form).
