You must deliver to the offices of the Arizona Corporation Commission, either in person or by mail, 2 copies of the document being served, an affidavit, declaration or certification of attempted service by a registered process server, and a check for $25.00. Credit card payments are not accepted over the phone; payment by credit card is accepted only if the documents are delivered in person to the A.C.C. offices.
