What is required for admission to Ollie Steele Burden Manor?
A potential resident’s current primary care physician will provide clinical information before admit. These items include: • a signed and dated 90-L form • a current chest x-ray • a current TB test • a current urinalysis • a current CBC result The resident (or resident sponsor) will complete an admissions packet and provide us with copies of a living will (if in existence), power of attorney (if in existence), and copies (front and back) of Medicare and insurance cards. Everything will be turned in to the admissions department and reviewed in advance of your desired admission date.
