Child support and/or health care coverage may be reinstated for a child under the age of 21 if a completed Child Attending School Written Confirmation Form (CSF 01 0353B) for the current term/semester is received by the CSP and proof the written consent was mailed to the parent ordered to pay support. Note: the child support order must have ORS 107.108 language and the support obligation for the child must not be terminated.
