Ramp permit application form signed by an authorized signature from your company on file in the ID office and an insurance certificate with at least $5,000,000 (five million dollars) general liability and $5,000,000 (five million dollars) auto liability. If applicable, you may also need a co-signature from the airlines, or if it’s for an OCIP contract, a co-signature from Linda Luzadder (206-835-5754).
