MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to: • Communicate with your doctor’s office. • Request medication renewals. • View your lab results released from your physician. • Communicate electronically and securely with your medical care team.
A. MyChart is your secure online connection to your medical record and your doctor’s office. With MyChart, you can: • Schedule, confirm, or cancel an appointment (24 hours notice is required to cancel an appointment) • Review past appointments and procedures • Request prescription refills • View information from your medical record including lab results and immunizations • Request referrals to specialists • Download a printable wallet—size card of your vital health information • Sign up for Proxy access • Update your personal information MyChart is not to be used for urgent matters. Please call your provider’s office for all urgent matters.
MyChart is a secure, online health management tool that connects CentraCare Clinic patients to portions of their personalized health information including: • Review past appointments • Request an appointment • View your health summary, current list of medications and test results as released by your provider • Access health information about a broad range of topics of personal interest • View your child’s immunizations • Receive messages when a new result is released to MyChart All you need to access MyChart is access to a computer, an email account and an Internet connection.
