With Green Shield, you no longer require a group number to process your claims. Your unique identification number includes an alpha prefix (UOT) that identifies your group enrolment. This prefix is followed by your nine digit identification number (your personnel number preceded by zeros to end up with a nine digit number), and ends with a unique two digit suffix. In addition, a unique identification number is also assigned to any dependants that are listed under your Family coverage. Please review the reverse side of your Green Shield ID card for a listing of your eligible dependants that are covered under our plan.
