What is meant by the Plan Specific Contract Number and how is it different from the Subscribers Social Security Number?
These two elements may be the same if the insurer uses the Social Security number to uniquely identify the coverage for the subscriber and all dependents. In that instance, the plan-specific contract number should be blank and only the subscriber’s Social Security number should be submitted. If the plan-specific contract number is required to be submitted, it is to be reported at the subscriber level and not at the individual person level.
