AGE Care Coordination is the name of a program within the Area Agency on Aging. LinkAGE can provide short-term, in-home assistance for persons who are 60 years of age and older, who need short term help to recover from illness or hospitalization. LinkAGE does not provide long-term services. We serve Hardin, Jefferson and Orange counties from this office.
