What is Geriatric Care Management and why have I never heard of it?
Geriatric Care Management solves the puzzle of fragmented health and social care systems. For example, the list of potential care providers for a frail elderly person grows as their dependence increases. Seniors often have several physicians, each with a specialty. They often take multiple prescription and over-the-counter drugs. The choices for home health and companion care are many and varied; the list goes on. Long-distance Caregivers and the elderly face a complex and fragmented support system. Geriatric Care Management is not well known because it is relatively new. Before 1950, the care provider system was much simpler, family members tended to live near each other and there wasn’t as much need for a knowledgeable intermediary like a Geriatric Care Manager. Much as we all might wish it, we are not likely to see those simpler times again and Geriatric Care Management is destined to become better known and widely used.
