The DS-2019 (formerly known as IAP-66) is the Department of State form that governs an individual’s J-1 program. The DS-2019 is issued by a Responsible Officer or Alternate Responsible Officer of the College or University as designated by the Department of State. The foreign national must additionally obtain “J-1 status” by entering the U.S. on a J-1 visa or obtaining an approved change of status from the Bureau of Citizenship and Immigration Services.
