well nursing care plans work the following way: Assessment-assessment of the patient and the history Diagnosis-nursing and medical diagnosis to get a framework of care Planning-plan your appropriate interventions and system of care Implimentation-begin your interventions Evaluation-see how your interventions are working and if anything needs to be changed, if so you begin at the beginning again
