What is Convalescent Care?
Convalescent Care (CC) is a short stay program governed under the Nursing Homes Act, Charitable Institutions Act and Homes for the Aged and Rest Homes Act funded under the Ministry of Health and Long Term Care. The convalescent care program is geared for clients who do not require acute (hospital) care, but are currently unable to return home because a period of convalescence is required to recover strength, endurance or functioning. Role of the Team The care team will work together with each individual client to complete assessments, develop goals, implement therapeutic interventions, and monitor progress to facilitate a successful convalescence.
