Community Care of Portage County (CCPC) is a part of the HHSD called a Care Mgt Organization, which administers the Family Care benefit to eligible Portage County residents. Family care is a flexible long term care that allows functionally/financially eligible seniors and adults with with a physical or developmental disability to remain as independent as possible in the residence of their choice.
