Refund requests should be submitted to the Columbia – St Mary’s Customer Service department at (414) 326-2100 (Physician Statements) or (414) 26-1900 (Hospital Statements). If no money is owed on other accounts, refunds will be issued within 90 days of receipt of request. Refunds will be issued with original method of payment.
