An Interdisciplinary Team is a group of people that includes the customer, their guardian or family members, the Service Coordinator, friends and people who provide services to them as identified in the Service Plan. This Team meets at least one time per year to develop the Service Plan. The Team may also meet whenever a Team member thinks there are important issues to discuss. Some Teams meet regularly and others only meet yearly. It’s up to the customer and their Service Coordinator.
