Since I now have one here is the official answer from an official, original, authoritative LdS source: From the 2006 “Church Handbook of Instructions Book 1: Stake Presidency and Bisphorics” p. 39 Melchizedek Priesthood Offices Stake President’s and Bishop’s Responsibilities The stake president oversees the conferral of the Melchizedek Priesthood and ordination of the the offices of elder and high priest. However, the bishop usually initiates recommendations for these ordinations. With the approval of the stake presidency, the bishop interviews the member as instructed on the Melchizedek Priesthood record form. Before doing so, he should carefully review the person’s membership record to verify that it does not include a comment about an ordinance restriction or unresolved Church discipline.
