An “appropriately completed” notice is a notice that includes the following information: • The name of an issuing state’s child support enforcement agency • The name and mailing address of the employee, who is enrolled or eligible for enrollment and who is obligated by a state court or administrative order to provide medical support for each named child • The name and mailing address of each child covered by the NMSN (The name and address of a state or local official may be substituted for the child’s name and address.) As long as the NMSN includes the information listed above, it should be considered “appropriately completed” even if some information is not included but is reasonably available to the employer. Note: All Illinois-issued NMSNs are pre-printed, contain all necessary information and should be considered appropriately completed.
