A. A “self-report” is an account from an operation of a serious incident that occurred at the operation or home. All regulated operations are required to make reports to licensing about certain types of incidents such as serious illness or injury to a child, criminal activity, unsafe building, grounds or equipment and other incidents that could pose a risk to children in care.
