A network of physicians and facilities with which Benesight or one of its clients has contracted to offer medical care at reduced rates. If you use a provider within the PPO network, your benefits will be paid at the highest level for your plan. If you choose to see a provider outside of the network, your benefits may be reduced or not covered depending on your plan. Please refer to the Schedule of Benefits section of your plan document for specifics.
