A notice of appeal is a written statement prepared by you that has the name of your case, the trial court docket number, and states your intention to appeal the judgment. The notice of appeal should have your name and the date of the judgment that you are appealing. See Exhibit 1. D. Do I have to do anything else when I file my notice of appeal? Yes. If there was a trial in your case in which people testified and evidence was taken, you will need to have a transcript of the trial.
