A NHDS is the state mandated form used for disclosure of natural hazards pursuant to the California Natural Hazard Disclosure Law. It lists all six geologic hazard zones along with a “yes”, “no”, or “do not know” box to be checked off verifying whether or not the subject property lies within one of the zones.
